How to request a Shared Lives service

If you are somone looking for a service, a care worker looking for a suitable service for someone you are supporting, or are a family carer wanting to look at service options for a member of your family, then this is what you need to know about requesting a service from Shared Lives South West.

You might then want to make a service request.  Please download the form relevant to the region or service you wish to request and send this into the office in your area.  All the details are on the forms.  At the moment you cannot fill the forms in online, but this will be updated shortly.
